To ensure the continued quality of care at HMP Warren Hill and HMP / YOI Hollesley Bay we’re looking for a Learning Disability Nurse (RNLD) / Resettlement Nurse to support us.

What do we need from you so you can do the job?

As a learning disabilities nurse or mental health nurse with current NMC registration, you'll collaboratively assess the needs of patients by planning, implementing and evaluating the care given and maintain records associated with this process. This may include the coordination of care of patients and liaising with other members of the multi-disciplinary team where necessary.
You will provide outstanding evidence based care, listening and acting on patient feedback so that care is personalised and informed by what matters to them. Focusing on both planned and unplanned care.
You'll actively encourage patients to work towards ownership over their health and wellbeing and will forge strong links with our partners and stakeholders, providing a service that people can trust, feel safe within and feel proud of.

This is a full-time role working 37.5 hours per week working cross-site at both HMP Warren Hill and HMP & YOI Hollesley Bay.

HMP YOI Hollesley Bay, is a Category D open prison holding males from 18 years on. It is built on a strong reputation in successfully preparing prisoners for final release. Spaced across 85 acres, it is situated in Woodbridge, Suffolk.

HMP Warren Hill is a category C adult male prison, located near the village of Hollesley in Suffolk (Hollesley, Woodbridge) - right near the coast and within 40 minutes' drive of Ipswich. We open Monday – Friday, 08:00-19:30 and weekends and public holidays 08:30-18:00. We provide a GP surgery provision alongside access to mental health and substance misuse services.
