Care UK is delivering a brand new model of care for general practice called Practice Plus. Practice Plus provides an enhanced general practice service to NHS patients at Brighton and Wandsworth, as well as telephone support and clinical appointments from Southall and Dudley. The service incorporates technology innovations and remote care which significantly improve patient access and provides opportunities to manage their health better.
Working with a leading Pharmacy Partner, Practice Plus provides enhanced general practice service to NHS patients through the use of technology, remote care and local clinical services delivered from our Pharmacy Partner sites.
We offer patients face-to-face or telephone appointments within 48 hours, and often on the same day. We also offer a wide range of approved NHS tests which can be done before the patients see a GP, ANP or nurse – improving their experience and often speeding up time to care.
About the Job
You will lead, and be responsible for the pharmacy support to Practice Plus based in the Clinical Hub either in Southall or Dudley. The role will be primarily based in Southall but travel to other sites will be required.
You’ll provide safe and effective telephone advice about medicines use and pharmaceutical care to clinicians, patients and carers accessing the service. We will also need you to provide prescriptions within the parameters set for the service.
You will assist the team in assessing and managing requests which relate to repeat prescription requests, medication queries, Health Information and minor ailments. As well as manage changes to medication which are required following hospital discharge or out-patient appointments.
You’ll work alongside GP’s, Practice Nurses, HCA’s and other non-clinical staff where you’ll be a valued member of a team who are focused on delivering innovation in patient care.

What we’ll need from you
•Be a qualified Pharmacist with valid PIN Number.
•Be an independent prescriber.
•Previous experience of prescribing post qualification of at least 12 months.
•Have an excellent understanding of community pharmacy services
•Previous experience of providing pharmaceutical advise to patients over the phone
