Job Summary

Provide safe and effective telephone advice about medication management, and pharmaceutical care to clinicians, patients and carers accessing the Integrated Urgent Care (IUC) service.

Assist the clinical hub team by appropriately assessing and managing referrals which relate to repeat prescription requests, medication queries, health Information.

Manage patient queries regarding repeat prescriptions and medicines within the pharmacist’s clinical area of expertise. This includes to provide appropriate self-care advice or purchase from a Pharmacy , referral to Pharmacy for emergency supply of medication , knowledge of local PURM services or if appropriate to manage without medication until own GP available. If necessary to refer to the prescribing GP/ANP within the clinical hub.

Work as part of the multi-disciplinary team in providing quality, evidence-based health care to meet the immediate needs of the patient.

Take an active role within the cross-disciplinary, multi-skilled team to ensure service quality in line with corporate goals and strategy and to assist in the development of new models for service delivery that have a positive impact on the health economy.

Skills & Experience

• Provide a holistic package of care to patients whilst working within a multi-agency team
• Good communication, listening and interpersonal skills
• Ability to work under pressure
• Articulate
• At least 2 years post qualification
• Experience and excellent understanding of community pharmacy services
• Experience of searching a range of information sources including electronic databases and the internet to retrieve
• Experience of working within a multi-professional / multi-agency environment.
• Understand the role of a Pharmacy Advisor
• Understanding of telephone advice triage service.
• Understanding of patient management in the Primary Care Setting
• Sound understanding of legal / professional implications of independent practice
• To be computer literate

Qualifications

• Registered as a Pharmacist with the General Pharmaceutical Council
• Registered Pharmacy prescriber
